Adding accident/incident sheets as a parent

Parents can create accident/incident sheets to inform the nursery about any accidents or incidents that occurred while the child wasn’t at the nursery.

Adding an accident/incident sheet through the eyparent app
  1. Tap the  icon in the bottom right of the eyparent app.
  2. Tap the  Pre-existing accident/incident form icon.
  3. Tap Add images to add supplementary media.
  4. Use the Body Map to point out the location of an injury, if necessary:
    1. Tap Add Body Map.
    2. In the pop-up window, select the body part that has been injured.
    3. On the next page, you can draw on the body part you have specified.
    4. Choose between Free Hand  drawing and using the Circle .
    5. Mark the injury on the image.
    6. Tapping Clear in the bottom left will erase any markings you make.
    7. Tap Save Picture or Done to add the body map.
  5. Fill out the rest of the sheet with details of the accident/incident.
  6. Tap Submit to share it with the nursery.
Adding an accident/incident sheet through the web portal
  1. Click on  Children in the sidebar.
  2. Click on the child you want to view.
  3. Click on the Accidents/Incidents tab.
  4. Click on the Add button in the top left.
  5. The Date and Time field is automatically filled in with the date and time you created the sheet. This can be edited.
  6. Click Attachments to add supplementary media.
  7. If necessary, use the Body Map to point out the location of an injury:
    1. Click the Select button next to Add Body Map.
    2. In the pop-up window, click on the body part that has been injured.
    3. Click Done.
    4. On the next page, you can draw on the body part you have specified.
    5. Choose between Free Hand drawing and using the Circle in the top left.
    6. Mark the injury on the image.
    7. Clicking Clear in the bottom left will erase any markings you make.
    8. Click Save in the bottom left when you’re done.
  8. Fill out the rest of the sheet with details of the accident/incident.
  9. Click Submit to share it with the nursery.
