Adding medicine forms as a parent

Parents can create and share medicine forms with nurseries to inform practitioners of your child’s condition, and how to administer the dosages.

Adding medicine forms through the eyparent app
  1. Tap the  icon in the bottom right of the eyparent app.
  2. Tap the  Medicine Form icon.
  3. The Date and Time field is automatically filled in with the date and time you created the sheet. This can be edited.
  4. Fill out the sheet with details of medicine administered. 
  5. Check the Temperature? box if you have checked the child’s temperature. Fill out details in the fields that appear.
  6. Tap Submit for Nursery Review to share it with the nursery.
Adding medicine forms through the web portal
  1. Click on  Children in the sidebar.
  2. Click on the child you want to view.
  3. Click on the Medicine Forms tab.
  4. Click on Add New Form in the top left.
  5. The Date and Time field is automatically filled in with the date and time you created the sheet. This can be edited.
  6. Fill out the sheet with details of medicine administered. 
  7. Check the Temperature? box if you have checked the child’s temperature. Fill out details in the fields that appear.
  8. Click Submit to share it with the nursery.
