You can use Accident/Incident sheets to record if a child has had an accident or incident that resulted in an injury, and what actions have been taken in response. Once it’s been filled out, the sheet can then be sent to the child’s parent.

You can create accident/incident sheets on the practitioner app.
  1. Log in as a practitioner to see the list of children you have access to.
  2. Tap a child’s picture to select them.
  3. Tap the  Accident/Incident Sheets icon at the top.
  4. Tap Add report in the top right.
  5. Use the Body Map to point out the location of an injury, if necessary:
    1. Tap Add Body Map.
    2. In the pop-up window, tap on the body part that has been injured.
    3. Tap Continue.
    4. On the next page, you can draw on the body part you have specified.
    5. Choose between Free Hand   drawing and using the Circle  in the top left.
    6. Mark the injury on the image.
    7. Tapping Clear in the bottom left will erase any markings you make.
    8. Tap Save Picture when you’re done.
  6. Fill out the sheet with details of the accident/incident.
  7. Tap Save as Draft, or Submit for Review
  8. If you have the correct permissions, you can Publish to Parent, who can then sign and mark the sheet as complete once they’ve seen it.
