You can use Medicine Forms to record whenever medicine needs to be administered to children. Once a form is filled out, it can be sent to the child’s parent.
You can create medicine forms on the practitioner app.
Log in as a practitioner to see the list of children you have access to.
Tap a child’s picture to select them.
Tap the Medicine Forms icon at the top.
Tap Add Medicine Form in the top right.
Fill out the sheet with details of the medicine to be administered.
Select the Form Type: Requested by Parent, or Administered by Nursery.
The Date and Time field is automatically filled in with the date and time you created the sheet. This can be edited.
Check the Temperature? box if you have checked the child’s temperature. Fill out details in the fields that appear.
Check the Parent/Carer Contacted? box if the child’s parent or carer has been contacted about the need to administer a dose of medicine. Fill out details in the fields that appear.
At the bottom of the form, under Record of Administration, tap on the Add Record button fill out details of each Dose given to the child.
If you have the correct permissions, you can Save and Start, which means the form will be marked In Progress, and parents will receive a notification asking them to view the form. If you're not a manager, you will have to Submit for Review so your manager can start the form.
When a form is In Progress, click Publish Dose to let parents see that a dose has been given to their child.
When a medicine form is fully filled out, tap Submit for parent review so the parent can confirm the form’s completion.
