Using accident/incident sheets

Accident/Incident Sheets allow you to record if a child has had an accident or incident, what kind, and what actions have been taken. The sheet can then be sent to the child’s parent. 

Adding an Accident/Incident Sheet on the practitioner app
  1. Log in as a practitioner to see the list of children you have access to.
  2. Tap a child’s picture to select them.
  3. Tap the  Accident/Incident Sheets icon at the top.
  4. Tap Add report in the top right.
  5. The Date and Time fields in the top right are automatically filled in with the date and time you created the sheet. These can be edited.
  6. Select the Report Type: Accident, Incident, or Pre-Existing. Pre-Existing refers to an accident/incident that happened outside the nursery. Parents can also fill this out themselves.
  7. Tap Add images to add supplementary media.
  8. Use the Body Map to point out the location of an injury, if necessary:
    1. Tap Add Body Map.
    2. In the pop-up window, click on the body part that has been injured.
    3. Tap Continue.
    4. On the next page, you can draw on the body part you have specified.
    5. Choose between Free Hand  drawing and using the Circle  in the top left.
    6. Mark the injury on the image.
    7. Tapping Clear in the bottom left will erase any markings you make.
    8. Tap Save Picture when you’re done.
  9. Fill out the sheet with details of the accident/incident.
  10. Click Save as Draft, or Submit for Review
  11. If you have the correct permissions, you can Publish to Parent, who can then sign and mark the sheet as complete once they’ve seen it. 
Adding an Accident/Incident Sheet on the web system
  1. Click on  Children in the sidebar.
  2. Click on a child to bring up their profile.
  3. Click on Accidents/Incidents.
  4. Click the Add button in the top left.
  5. The Date and Time field is automatically filled in with the date and time you created the sheet. This can be edited.
  6. Select the Report TypeAccident, Incident, or Pre-Existing. Pre-Existing refers to an accident/incident that happened outside the nursery. Parents can also fill this out themselves.
  7. Click Attachments to add supplementary media.
  8. If necessary, use the Body Map to point out the location of an injury:
    1. Click the Select button next to Add Body Map.
    2. In the pop-up window, click on the body part that has been injured.
    3. Click Done.
    4. On the next page, you can draw on the body part you have specified.
    5. Choose between Free Hand drawing and using the Circle in the top left.
    6. Mark the injury on the image.
    7. Clicking Clear in the bottom left will erase any markings you make.
    8. Click Save in the bottom left when you’re done.
  9. Fill out the sheet with details of the accident/incident.
  10. Click Save as Draft, or if you have the correct permissions you can Publish to Parent. The parent can then sign and mark the sheet as complete once they’ve seen it.
